Remember to bring the following when you come in for your appointment

• Drivers license or valid state ID
• Referral letter from your Primary care physician, family physician or other doctor (If required).
• Have your insurance information with you.
• Bring copies of records and x-rays, MRIs, CT scans, etc. from clinics outside.

You may be able to avoid repeating a study if your information is here. You may also arrange to have the records sent here prior to your appointment. Also, please dress so that your injury area can be most easily examined and/ or x-rayed.

To cancel an appointment:

Telephone the office during business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
